PCP Claim Options

You may submit PCP claims by mail, fax, or EDI.

To submit your facility claims through your Clearinghouse, use the WellCare Medicare Payer ID: 14163.

Referral/Authorization Information

  1. Go to the WellCare Provider Portal.
  2. Log in using your provider credentials (or register if you are a new user).
  3. Once logged in, navigate to the Care Management or Authorizations section.
  4. Select “Create New Authorization” or the appropriate referral submission option.
  5. Enter all required member information (Member Name, Member ID, Date of Birth).
  6. Enter the required clinical information, including diagnoses, CPT/HCPCS codes, and service dates.
  7. Upload supporting documentation (charts, clinical history, notes) if indicated.
  8. Review and submit the request. A reference number is typically provided once the submission is accepted.
